The throbbing, pulling or creeping sensations of Restless Legs Syndrome (RLS) can be miserable—and affect at least one third of all women with fibromyalgia. These uncomfortable feelings and urge to move the legs are often triggered by trying to relax or sleep, making it hard to fall and stay asleep. And it is not just the legs, some people experience the restless sensations in their arms or over their whole body! Since sleep is already a huge issue in fibromyalgia, it is really important to be look for and treat any condition that is making sleep worse.

If you answer yes to any of these questions, you may have Restless Legs Syndrome:
o Do you often get a strong urge to move your legs to relieve unpleasant sensations?
o Are the symptoms worse when you’re sitting still or lying down and resting?
o Do you get temporary relief from moving your legs or walking?
o Do the symptoms start or get worse in the evening or at night?

There are definitely medications that can help ease these symptoms, but many of them have significant side effects. Some very effective natural treatments for Restless Legs Syndrome to consider are iron, magnesium and cannabidiol (CBD).

Iron for Restless Legs
We know that restless legs is caused by low levels of the brain chemical dopamine, and many of the prescription treatments for RLS act to increase dopamine levels. Iron is a key player in the production of dopamine, so low iron levels in the brain may lead to low dopamine levels and restless legs symptoms. There has long been a suspected connection between low iron levels and RLS, and people with iron deficiency anemia often experience restless legs symptoms. However people with normal iron blood levels can still experience RLS, and it appears that it is the level of iron that reaches your brain that matters, not the amount floating in your bloodstream. One of my patients with severe RLS and normal blood iron levels had her symptoms eliminated after a series of intravenous (IV) iron infusions, which is the most effective way to get iron into the brain. One study found that IV iron infusions showed significant improvement of RLS symptoms in 76% of the subjects without any major side effects.

For patients with RLS I aim to get their ferritin levels (a blood test for iron) around 70 to maximize how much iron is getting into their brain. Typically any ferritin above 10 is considered “normal”, but if you have RLS it is worth a try to get your ferritin levels up into “optimal” range and see if symptoms improve. In my clinical experience, the results can be pretty dramatic, but be sure to talk with your health care provider before starting iron supplementation as your levels will need to be monitored to ensure they don’t get too high as that can be harmful. You want to find the amount of iron supplementation that keeps your ferritin around that optimal level of 70.

Some forms of iron can be hard on the stomach and cause nausea and constipation. I have found iron bisglycinate to be the best tolerated, my favorite is Opti-feren C by Pure Encapsulations. To optimize iron intake make sure to take it away coffee, tea, and calcium supplements as these can interfere with iron absorption. I often recommend taking any iron with dinner as most of us are not drinking coffee with dinner!

An alternative approach for those that don’t tolerate or absorb oral iron is to apply it transdermally. Yes, we can absorb some iron through our skin! Compounding pharmacies can make a prescription iron cream, although a few of my patients found it discolored their skin. For topical iron I prefer the PatchMD Iron Plus Topical Patch as it is less messy, cheaper, and has no skin discoloration issues.

In addition to getting ferritin levels up to optimal range, for those with severe RLS I will often add some specific nutrients that increase the ability of the body to utilize iron, like the active forms of B-6, B-12, and folate, found in MethylGuard by Thorne Research. For this purpose I have patients just add one capsule of MethylGuard per day.

Magnesium for Restless Legs
Magnesium is another beneficial mineral that can help RLS. Clinically I see this all the time and this study found it was effective for mild to moderate symptoms. Magnesium promotes muscle relaxation and soothes irritable nerve endings. The best form of magnesium for this purpose is magnesium glycinate 240mg to 480mg at bedtime. Magnesium glycinate is the type least likely to cause loose stool, one common and undesirable side effect of magnesium supplementation. Just like iron, we can absorb magnesium through our skin, so you can also apply magnesium oils and lotions directly to your legs. Remember that magnesium is a salt, so some oils can be irritating to the skin. My goal with Frida Botanicals magnesium cream was to make a potent cream that was still gentle and moisturizing. Some of my patients have found that a combination of oral and topical magnesium is the ticket to ease their restless legs symptoms while limiting the laxative effect.

Cannabidiol (CBD) for Restless Legs
Many patients have reported that cannabis (marijuana) eases their restless leg symptoms. This small study found 6 patients reported complete remission of their RLS with cannabis. Cannabis contains lots of different chemicals, so it is unclear which are specifically giving benefit for RLS, but CBD (cannabidiol), is suspected to be the key component of cannabis for RLS. In fact an Israeli company is currently studying CBD as a treatment for Restless Legs Syndrome.

CBD has strong muscle relaxant and nerve calming effects which can really ease the discomfort of restless legs. And just like iron and magnesium, CBD can be taken orally or applied topically to the legs. I have seen patients get improvement from CBD applied topically or a CBD tincture under the tongue before bed. And unlike cannabis, CBD does not have any psychoactive or mind-altering effects and if it is derived from hemp it is legal and accessible in all 50 states. Can Marijuana Treat ADHD?

Marijuana is sometimes used as a self-treatment by individuals with attention deficit hyperactivity disorder (ADHD).

Advocates for marijuana as an ADHD treatment say the drug can help people with the disorder handle some of the more severe symptoms. These include agitation, irritability, and lack of restraint.

They also say that marijuana has fewer side effects than traditional ADHD medications.

Laws and research

Marijuana remains illegal at the federal level. Each year, more U.S. states have passed laws allowing the sale of marijuana for medical purposes. Some states have legalized it for recreational purposes, too. Many states still outlaw any use of marijuana. At the same time, research into the effects of the drug on health conditions and diseases has increased. This includes research on marijuana use in individuals who’ve been diagnosed with ADHD.

Online health forums are filled with comments from people saying they use marijuana to treat symptoms of ADHD.

Likewise, individuals who identify as having ADHD say they have few or no additional issues with marijuana use. But they aren’t presenting the research on adolescent use of marijuana. There are concerns for the developing brain’s learning and memory.

“Many adolescents and adults with ADHD are convinced that cannabis does help them and has fewer side effects [than ADHD medications],” says Jack McCue, MD, FACP, an author, physician, and emeritus professor of medicine at the University of California, San Francisco. “It may be that they, not their doctors, are correct.”

Dr. McCue says he’s seen patients who report classic marijuana use effects and benefits. They report intoxication (or being “high”), appetite stimulation, help with sleeping or anxiety, and pain relief, for example.

Dr. McCue says these people sometimes report effects that are often seen with typical ADHD treatments, too.

“The limited research on what patients say cannabis does for ADHD symptoms indicates that it is most helpful for hyperactivity and impulsivity. It may be less helpful for inattentiveness,” Dr. McCue says.

Research in 2016 analyzed some of these online threads or forums. Of the 286 threads the researchers reviewed, 25 percent of posts were from individuals who reported that cannabis use was therapeutic.

Only 8 percent of posts reported negative effects, 5 percent found both benefits and harmful effects, and 2 percent said using marijuana had no effect on their symptoms.

It’s important to remember that these forums and comments aren’t clinically significant. They’re also not evidence-based research. That means they shouldn’t be taken as medical advice. Talk with your doctor first.

“There are descriptive accounts and demographic surveys that report that individuals with ADHD describe marijuana as being helpful in managing inattentiveness, hyperactivity, and impulsivity,” says Elizabeth Evans, MD, psychiatrist and assistant professor of psychiatry at Columbia University Medical Center.

However, Dr. Evans adds, “while there certainly may be individuals who experience benefit in their symptoms of ADHD, or those who are not adversely impacted by marijuana, there is not sufficient evidence that marijuana is a safe or effective substance to treat ADHD.”

Cannabidiol (CBD) is also promoted as a helpful treatment for individuals with ADHD.

CBD is found in marijuana and hemp. Unlike marijuana, CBD doesn’t contain the psychoactive element tetrahydrocannabinol (THC). That means CBD doesn’t produce a “high” the way marijuana does.

CBD is promoted by some as a possible treatment for ADHD. Dr. McCue says that’s because of “anti-anxiety, antipsychotic effects of CBD.”

However, “the lack of a potential paradoxical benefit from the stimulating effects of THC make CBD theoretically less attractive,” he says.

Dr. Evans adds, “There are no large-scale clinical trials looking at CBD for ADHD. It is not considered an evidence-based treatment for ADHD at this time.”

Some people with ADHD report that they find therapeutic benefit from using marijuana. Despite these anecdotal stories, research doesn't support marijuana, or CBD, as a treatment for ADHD. Who's right?

What to Know About Synthetic Marijuana (Fake Weed) Use

Synthetic cannabinoids, also called synthetic marijuana or fake weed, have been used by many as an alternative to marijuana since products were first introduced in 2002. Despite the fact that these man-made products were created in laboratories to help scientists study the cannabinoid system in the human brain, they often claim to be made of “natural” material from a variety of plants.

Hundreds of synthetic cannabinoids exist and the effects can be unpredictable and even life-threatening.

Also Known As: There are countless fake weed products being sold as herbal smoking blends, legal bud, herbal smoke, marijuana alternatives, fake weed, or herbal buds. This makes it difficult for parents and other adults to identify them. Some of the brand names include Blaze, Blueberry Haze, Dank, Demon Passion Smoke, Genie, Hawaiian Hybrid, K2, Magma, Ninja, Nitro, Ono Budz, Panama Red Ball, Puff, Sativah Herbal Smoke, Skunk, Spice, Ultra Chronic, and Voodoo Spice.

Drug Class: Synthetic marijuana products are classified as new psychoactive substances (NPS), or unregulated mind-altering substances intended to produce the same effects as illegal drugs.  

Common Side Effects: Side effects of the drug include elevated mood, relaxation, altered perception, symptoms of psychosis, extreme anxiety, confusion, paranoia, hallucinations, violent behavior, suicidal thoughts, rapid heart rate, raised blood pressure, vomiting, kidney damage, and seizures.

How to Recognize Fake Weed

Synthetic marijuana often contains a mixture of dried leaves from traditional herbal plants. They are various colors, including green, brown, blonde, and red, and often sold in small packets approximately two by three inches. The packets are often colorful foil packs or plastic zip bags. Some online sellers of legal fake weed products do so with disclaimers like “not for human consumption.”

What Does Synthetic Marijuana Do?

Fake weed works on the same brain cell receptors as THC or delta-9-tetrahydrocannabinol (the psychoactive ingredient in marijuana that gets you high). It is typically smoked, brewed in tea, or vaped.   Many of these products are legally marketed as “herbal incense” or “potpourri”.

Some people who use herbal buds say that it produces a high similar to that of marijuana, but it doesn’t last as long. Others experience a relaxed feeling, rather than the “head high” that real marijuana produces. Also of note is the “harsh” taste, which people say “makes your throat burn and your lungs ache” long after you smoke.

Since there are no standards for making, packaging, or selling synthetic weed, it’s impossible to know the type and amount of chemicals in each product as well as what the fake weed will do to you.

What the Experts Say

Although they are often marketed as “100% organic herbs,” none of the fake weed products on the market are completely natural. They have all been found to contain various synthetic cannabinoids, or chemicals produced in laboratories.

Originally, fake marijuana products contained a chemical called HU-210, which has a molecular structure very similar to THC. Because HU-210 is listed as a Schedule I controlled substance in the United States, these fake weed products were manufactured and sold only in Europe.

Since then, new synthetic cannabinoid agonists have been created. They are too numerous to list. Some are similar in structure to THC; others are not. Some are classified as controlled substances. By using different synthetic marijuana mixtures, manufacturers are able to continue to legally market their products in the United States when another formulation becomes illegal.

According to the DEA, the majority of these chemical compounds are produced in Asia with no regulations or standards.   They are then smuggled into the United States where they are sprinkled onto “plant material,” packaged and ultimately sold in tobacco shops, convenience stores, and the like.

Some of these chemicals are still legal. However, since synthetic marijuana first hit the market, more than 20 of these compounds have become controlled in some way at the federal level.   At the same time, they noted that more than 75 additional compounds have been identified but are not currently controlled.  

In 2015, the DEA listed 15 varieties of synthetic marijuana as Schedule I controlled substances in the Drugs of Abuse resource guide. This places them in the same federal category as heroin, crack cocaine, and marijuana.

Many people buy into the idea that fake marijuana products are safe since the chemicals are “legal” and contain “natural” ingredients. However, this has proven to be false with multiple cases of severe, unexplained bleeding or bruising, and some deaths.   Other reports show an increase in emergency room visits due to rapid heart rate, vomiting, violent behavior, suicidal thoughts, kidney damage, and seizures.

Off-Label Uses

Some of the fake marijuana products sold commercially claim to contain herbs traditionally used for medicinal purposes, including:

  • Beach bean (Canavalia maritima)  
  • Blue Egyptian water lily (nymphaea caerulea)
  • Dwarf skullcap (scutellaria nana)
  • Indian warrior (pedicularis densiflora)  
  • Lion’s tail (leonotis leonurus)
  • Indian lotus (nelumbo nucifera)
  • Honeyweed (leonurus sibiricus)

However, one study revealed that some of the herbal ingredients listed by the manufacturers could not be found in the products.

Beyond the synthetic cannibinoid HU-210, which is used by scientists to identify cannibinoid receptors in the brain and study the effects Δ-9-tetrahydrocannabinol (Δ-9-THC), there are no approved or off-label medical uses for synthetic marijuana.

Common Side Effects

While research is advancing, the effect synthetic marijuana products may have on the human body is largely unknown. To date, few studies have been published testing the effects of the chemicals on users. Within the DEA report, they note overdoses that have caused fatal heart attacks.   Similarly, acute kidney injury resulting in hospitalization and dialysis have been connected to these synthetics.

One study compared the level of impairment for drivers who were arrested for intoxicated driving.   One group had smoked synthetic cannabinoids and those in the other group were high on marijuana. The study found a significant increase in confusion, disorientation, and incoherence in the synthetic marijuana group. Slurred speech, a side effect not normally associated with natural cannabis use, was also reported among the synthetic cannabinoid users.

Beyond the short-term effects mentioned, an increase in blood pressure, as well as seizures, tremors, and anxiety, have been noted in synthetic marijuana users.

Whether these observed symptoms will have lasting effects, particularly on adolescents and young adult users, is not yet known. Of course, smoking any substance could have negative effects on the lungs.

“The problem with JWH-018 (a synthetic cannabinoid compound) is that absolutely nothing is known regarding its toxicity or metabolites,” says John Huffman, who helped develop the JWH-018 chemical. “Therefore, it is potentially dangerous and should not be used.” JWH-018 is also known as 1-Pentyl-3-(1-naphthyl) indole and is one of the Schedule I controlled substances listed with the DEA.

Recently, a version of synthetic marijuana was laced with rat poison, causing uncontrolled bleeding in hundreds of people and killing several others who ingested the tainted products.

If you or a loved one has used synthetic marijuana and begin experiencing severe, unexplained bleeding or bruising, call 911 or asked a loved one to take you to the hospital immediately. These are all signs of contaminated cannabinoid products.

Signs of Use

If you are a parent of a young adult, it pays to know the behaviors and physical effects of using fake weed. While exhibiting one or two of these signs might not mean that your child is using, they are all strong indicators of drug use and should be taken seriously.

  • Burning incense
  • Buying or using eye drops
  • Possessing dried plants or herbs
  • Having rolling papers or vape pens
  • Receiving suspicious packages in the mail
  • Displaying unusual or secretive behaviors
  • Restlessness  
  • Red or irritated eyes
  • Pale complexion  
  • Acting confused

Myths and Common Questions

Perhaps one of the biggest misconceptions about herbal bud is that it is “natural marijuana.” It is not; it is created from any of several hundred man-made synthetic chemicals that are sprayed onto the chopped plant material.

Synthetic marijuana is also far more potent, containing TCH analogs or synthetic cannabinoids that can be up to 600 times more potent than THC found in marijuana.   Often, additives, toxic impurities, and other types of drugs are also found in fake weed products.

Tolerance, Dependence, and Withdrawal

Regularly using “fake weed” can result in increased tolerance, or needing more and more of the drug to experience the same high. If you regularly use synthetic cannabinoids, you can also become both physically and psychologically dependent. This means if you stop abruptly, you’ll likely experience withdrawal symptoms.

Since the chemical composition of fake weed is unknown and can change from batch to batch, tolerance, dependence, and withdrawal may also vary.

How Long Does Fake Weed Stay in Your System?

How long synthetic cannabinoids stay in your system depends on several factors, including the type, how it is administered (i.e., inhaled or ingested), amount consumed, and frequency of use. Since these synthetic drugs don’t trigger a positive result on most standard urine drug tests   , many people turn to these drugs in an attempt to avoid positive drug screens for employment, rehab, or legal reasons.

Addiction

Long-term, regular use of synthetic cannabinoids can lead to addiction. If you have a history of mental illness or a substance use disorder, the risk of addiction is even greater.

In addition to building up a tolerance and experiencing symptoms of withdrawal, other signs of synthetic cannabinoid addiction can include:

  • You use more than intended, even after telling yourself that you’ll only “take a few hits.”
  • You are unable to cut down or stop and have likely failed numerous times at quitting.
  • You spend lots of time getting high, often at the expense of spending time with loved ones or doing activities you once enjoyed.
  • You continue to use despite any problems with family and friends, employment, or legal troubles.
  • You depend on the drug to “relax” or for creativity.

Withdrawal

Symptoms of synthetic weed withdrawal can range from mild to severe, depending on how frequent and how long you have been using, and include the following:

  • Headache  
  • Severe anxiety
  • Depression  
  • Irritability

How to Get Help

If you suspect that someone you love is using synthetic marijuana, the most important thing you can do is spend time with them, communicate the dangers of fake weed, and watch for any signs of use. While behavioral therapies and medications have yet to be specifically tested for the treatment of synthetic cannabinoid addiction, a health care professional can work with you and your loved one to safely detox from the drug as well as identify and treat any co-occurring mental illness.

Can marijuana help treat ADHD?

Research on the consequences and effectiveness of using marijuana to treat children, teens, and young adults with attention deficit hyperactivity disorder continues to evolve.

However, the results are mixed and do not advocate the use of the drug for this condition on a medical basis.

Medical marijuana is still illegal in many states, and research has not proved its suitability for treating attention deficit hyperactivity disorder (ADHD).

This article looks at the scientific research and other evidence for and against the potential use of marijuana as a treatment for symptoms of ADHD.

ADHD is a neurodevelopmental disorder that affects around 6–9 percent of children and young adults, and about 5 percent of adults, globally.

A person with ADHD might find it hard to focus on tasks, frequently fidget, show signs of restless behavior, and they may be unable to stay still or quiet at appropriate times.

ADHD can lead to people having relationship problems or difficulties with academic work at school and college despite having normal or superior intellectual abilities.

Treatment for ADHD usually involves doctors prescribing stimulant medications, such as Ritalin or Adderall.

These medications are believed to help correct the levels of a neurotransmitter in the brain called dopamine. However, they may have unpleasant side effects.

To avoid these adverse effects, some people with ADHD use marijuana as a treatment option. This is because marijuana is believed to have the same impact on dopamine levels as prescription medications.

However, there remain many unanswered questions about how useful it is, and its safety, especially for children and young people.

Supporters of marijuana often claim it is a safe drug and has no risks of addiction. But opponents call it a “gateway drug,” potentially leading to the use of other drugs, and they claim it is more dangerous than some realize.

Marijuana is one of the most commonly used recreational drugs in the U.S. and is popular among younger adults. Most people smoke or eat the plant to produce a “high.”

In recent years, marijuana has made news as an alternative treatment for a variety of health conditions, including pain and mental health problems.

Despite these facts, questions and doubts remain.

A study of 268 separate online discussion threads reported that 25 percent of people said they believed that marijuana had a positive role to play in ADHD symptom management. The study points out, however, that research proving a connection between marijuana the management of ADHD is limited.

Some schools of thought suggest that ADHD migt stem from a lack of dopamine in the prefrontal cortex region of the brain.

As a neurotransmitter, dopamine is a chemical that transmits signals between nerve cells in the brain. Dopamine is believed to affect thought processes, including memory and attention.

Substances in recreational drugs, such as marijuana, lead to the production of more dopamine in the reward center of the brain.

The brain’s reward center gives an individual a pleasant sensation when they use marijuana and other drugs. However, this cycle of recreational drug use and increased dopamine can lead to the development of dependence.

Nature journal published a study in 2017 that discusses the dopamine-releasing action of tetrahydrocannabinol (THC), an active chemical component in marijuana, and the source of its pleasure sensation. The researchers advised that THC boosts short-term dopamine levels but may dull the system that releases dopamine long-term.

This varying effect suggests that even if marijuana provides short-term symptom relief, better focus, or sedation for people with ADHD, longer-term use may result in more harm than good.

However, in the journal Brain, an imaging study by researchers at Cambridge University in the United Kingdom disputed the connection between ADHD and dopamine. They, instead, related ADHD to the structure of the gray matter in the brain.

One trial from 2017 tested a cannabinoid medication on people with ADHD. While the participant number was small and the findings were not statistically significant, the results did show small improvements in ADHD symptoms. The results also suggested that adults who take cannabinoids for ADHD experience the side effects less than children.

A further review of available clinical evidence on marijuana cited a case that — although not involving ADHD — showed that a child with autistic spectrum disorder had improved hyperactivity levels after receiving a cannabidiol treatment.

However, other research shows a relationship between marijuana dependence and ADHD.

One study of 99 people seeking treatment for cannabis use disorders showed an estimated ADHD prevalence of between 34 and 46 percent.

Overall, research seems to indicate that, although marijuana seems to alleviate certain short-term effects of ADHD, it may present a heightened risk of dependence and might even make ADHD worse.

Is medical marijuana available for ADHD?

People who use marijuana as a treatment for ADHD often self-medicate, which means a doctor does not prescribe or recommend the marijuana they take.

The evidence for medical professionals to recommend or prescribe marijuana as an active treatment for ADHD is not compelling enough at present.
